Overview:  The Community Health Worker will act as a member of a Community Public Health Team (CPHT) to mobilize the community to identify, highlight, and address challenges to community health. Raise awareness, provide health related and social service-related outreach, resources, and referrals to community members to address healthcare needs.

 

Duties and Responsibilities: Reporting to the Outreach and Prevention Program Manager, the Community Health Worker will: Community Education and Outreach - Provide door-to-door outreach and education to community members including annual household visits. Support vaccination efforts including mobile clinics and other events. Offer health screenings and household assessments and ensure residents are connected to healthcare, mental health, or other social service-related services. Organizes a minimum of two community convenings each year and participate, as appropriate, in other community convenings to share data, jointly identify health issues, and develop community action plans. Community Mobilization: Mobilize partnerships to advance health equity and improve community conditions. Develop- Create program resources, including educational materials and recruitment/marketing materials, as directed by supervisor. Data Collection and Documentation – Conduct data collection during home health assessments and provide linkages to Case Mangers. Will implement evaluation activities, collect, and enter data and monitor evaluations to ensure program impact. Completes required reports for funders, responsible for meeting grant milestones. Recruitment - Assists in recruitment and selection of AmeriCorps volunteers and for SU internships and prevention education advocate role, participates in training of staff. Training and Workshops: Builds relationships and represents Strength United by participating in community events, health, safety, and resource fairs; attend skill-based training, on-going technical assistance, and/or networking opportunities. Performs other duties as assigned.

 

Qualifications:  Minimum of a High School diploma preferred. Two years of volunteer or work experience in public health, community organizing, or in a social service setting is preferred and one year of outreach experience. Computer competency, including Word, Excel, Outlook and PowerPoint. Bilingual Spanish Speaker required.

 

Knowledge, Specialized Skills, and Abilities:  Working knowledge of the principles, practices, and trends of community education and advocacy issues in California, and especially in the San Fernando and Santa Clarita Valleys. Ability and specialized skills to motivate and teach others, lead discussions, and generate new ideas; speak and write effectively; write narrative and statistical reports; carry out a variety of professionally complex assignments without detailed instructions; work flexible hours, including evenings and weekends; work comfortably with multimedia materials; establish and maintain cooperative working relationships with a variety of individuals and organizations; and relate well with a broad spectrum of persons of varying socioeconomic status, culture, ethnicity, and lifestyle. Computer and organizational skills necessary. Must demonstrate an acceptance and respect for cultural diversity in all its forms, including ethnicity, sexual orientation, abilities, and religious backgrounds and an ability to work collaboratively in an ethnically linguistically, and culturally diverse environment.

Physical Requirements and Work Environment:

Work is performed in an office setting with a low-to-moderate noise level; employee will need to utilize their personal vehicle for travel to and from meetings outside the office; employee must regularly communicate by telephone, written communications and in-person with staff, families served, partners, funders and other professionals; occasionally lift, carry and position objects weighing up to 20 pounds. Pay, Benefits, & Work Schedule

This position is employed through The University Corporation (TUC).

The anticipated hiring range: $19.71 - $23.33/hour

Time Base: Full-Time
